Mark J. Smith, M.D. (Young Investigator 2001) of National Institute of Mental Health (NIMH/NIH), is using a technique called paired-pulse transcranial magnetic stimulation (pTMS) to test the relationship of steroids and excitability of the cortex in patients with obsessive compulsive disorder (OCD). The effects of a steroid, pregnanolone, will be evaluated. Changes in cortical excitability in patients with OCD and healthy controls will be monitored before and after an intravenous challenge with pregnanolone. Steroids such as pregnanolone have been shown to have powerful effects on the GABA system, and patients with OCD have evidence of a deficiency of this neurotransmitter.
